Suspension-Modification Procedure
Instructions for deleting-modifying an End User Account:

The online application should be completed in one sitting. It is not possible to save a partially completed application and come back to it at another time. We would recommend that you allow at least 10 minutes to complete it.
The online application concerns end users, who have stopped providing their services, as they were.

The electronic application must be completed by a person who will request the deletion-change of the account for your business.
Before you start filling out the online application, make sure you have all the necessary information available to complete it, such as:
- Former Company Name
- Form Completion Date when registering the company’s previous name
- Date of Company Name Change (if any)
- New Company Name (if any)
- Comments
- Name of Applicant
- Company Contact Phone
- Company Email Address (unique email for the System)
- Application registration date
